This is yet another insurance policy from ING Vysya Life Insurance that has been designed to offer insurance cover to the insured against accidental disability. To make such a claim the insured must notify this within thirty days from the day in which the incident has taken place.
The most important thing to do while making such a claim is to get a claim form either from the nearby office of ING Vysya Life Insurance or the official website of the insurance company from where the form can be downloaded at free of cost. Some other documents are also needed along with the form that acts as a proof about the incident that the insured has faced.
The documents that are needed for making such a claim are as follows:
- The first and foremost document that is needed for making such a claim is the claim form.
- The original documents of the policy are also needed for making such a claim.
- The discharge summery from the respective hospital must also be attached along with the claim form.
- Case sheets of the hospitals are also needed for making such a claim and those must be attested by the respective hospital authority.
- All the medical reports must also be submitted along with the claim form. Here medical reports are referred to the X-Ray reports, MRI, CT scan, Pathology Reports, Angiography Reports, etc.
- The insured must also present a copy of the identification document and that must contain his or her photograph.
- The residential address proof of the insured must be submitted. If in the mean time the insured have shifted to another place then he or she must inform about this matter to the insurance company.
- An attested copy of the FIR that has been made at the local police station.
These are the documents that an insured need to submit to the insurance company while making such a claim.
