I want to make my passport but I have lost my dateof birth proof . I have my original pan card is it possible for me , I can make my passport , if yes Pls tell me the details.

As i need to apply for new passport for my wife since she does'nt have reation card seperately just 3 years before we got married but i got a postal Id proof card for my wife in present address and she as a bank account in present address other than that she has mark sheet, communitte certificate too is this ok to apply for new passport in my present address, pls do help in this problem.

Help !! Normally, a person will have Name + Father's Name + Surname. As a south indian, i dont have a Surname / Village name. For Date of Birth proof I submitted my 12th Std Certificate. For Education Proof i submitted my B.Com certificate. Both 12th std certificate and B.Com certificate had my name + my father name as INITIALS (Eg. PREMA B).
For Address Proof, I submitted copy of IT Assessment (2009), Gas Connection Bill, and Ration Card. Additionally i gave PAN Card & Driving Licence - all these documents showed my name + my father's complete name ((Eg. PREMA BALKRISHNAN).
However, the officer scrunitizing my record rejected it saying that an Affidavit showing PLACE OF BIRTH Proof is required and a Bank statement is also required with my name + my father's name.
(a) Nowhere in the Passport Website (passport.gov.in) it is written that Place of Birth proof is required. There is no Affidavit format for Place of Birth available on the Passport website. Even the Annexure A Format states that it is meant for Illiterate People who do not have any other document to give for Date of Birth. (b) Bank Statement, IT Assessment etc. are required for Address proof and Not Name Proof. I dont understand why Bank statement is required when other documents (like IT Assessment) were submitted. Kindly guide me if there is any other documents to be produced. I am still not sure why my application was rejected and I dont want my application to get rejected the next time.... Pls help !!!

I do not have any ORIGINAL copy for Date of Birth Proof (Birth Certificate, Leaving Certificates etc). The only originals that are available stating my DOB are 10th std Marksheet, FYJC & SYJC Bonafied Certificates & PAN Card. Will it be accepted by authorities as a valid proof. (my DOB is 13.3.76).
