You would be required to fill this attached form in all respect and attach a passport size photograph of your on to this form and deposit it at the nearest State Bank of India branch or at the branch where you wish to open your Public Provident Fund account.

The rest of the procedure remains the same for a Public Provident Fund account with the post office. Post the submission of the form, the account would be opened for you. Also a pass book would be provided to you, where in all the updated balances and the interests credited to your account as per the policies of the government would be updated from time to time. You would not be required to do any run around in case you have a question.
