Obtaining the Divorce decree

I have an affidavit on mutual divorce duly signed by my ex-wife and mentioning that she will not ask for any alumni or other stuff and that we have given divorce to each other as per the muslim law infornt of a Mufti all of these documents are endorsed by the notary now if I need a divorce decree what else should I do can any one guide me.....
