revered sir/Madam sub; misplaced of policy documents request rec ref; my policies no 1-646246359 2- 646264434 iam submit to your good self that iam rampaali nivedana lic policy holder bearing the policy no 1-646246359 2- 646264434 at lic mehd-patnama hyderabad sir/madam my policy document has been misplaced during my house shifting due to finical problem i am not paying the premium reguary since I reqest your good self kindly intruct the concerned branch no refund the amount for which i am eligible at the earliest
